Waiting list re-opened with Foria Clinic
Connect-Clinic is excited to announce we have joined PurposeMed to establish a sustainable home for virtual gender-affirming care.
Our new service, now called Foria Clinic, has launched in Ontario and will be accepting new patients for gender-affirming services.

Changes to Virtual Health Care Beginning December 1, 2022
As you may know, the Ministry of Health has finalized changes to the Physicians Service Agreement which changes the virtual care billing codes. Because of this, Connect-Clinic will not be able to see new patients by virtual means beginning December 1, 2022. In a recent update, the Ministry of Health has decided to not allow our physicians to bill for care for existing patients. As a result, we are pausing routine clinical care via virtual visits as of December 1, 2022. Our wait-times will increase while we continue to advocate to reverse this change and secure funding in order to continue clinical care.
If you are on the waitlist, we will reach out to you to determine how we can move forward. Please do not reach out to us at this time – we are overwhelmed with questions about the waitlist.
For our existing patients, we will continue to provide prescription renewals and manage any urgent issues. If you have an appointment scheduled in December, we will be in touch with you directly.
What happens next?
Registered Patients: If you are an existing patient who is in need of a prescription renewal, please ask your pharmacy staff to fax a renewal request to 437 317 9843 before you run out of your medications. Prospective Patients on the Waitlist: Please allow us to reach out to you as we gather more information about how we can move forward. We are currently care planning with out existing patients.

Resources for Individuals
If your primary care provider would like some advice around gender affirming care from a specialist, they can contact the Provincial Transgender Group via the Ontario Telemedicine Network's eConsult service.
If you do not have a primary care provider, you can register for Health Care Connect for assistance in connecting with a doctor or nurse practitioner in your community. Health Care Connect cannot guarantee there will be a provider near you offering gender affirming care.
Rainbow Health Ontario's service provider directory is a good resource to find a local provider. If you cannot find a provider in your area within the provider directory, you can contact info@rainbowhealthontario.ca. Rainbow Health Ontario cannot guarantee there will be a provider near you offering gender affirming care.

Resources for Primary Care Providers
Primary care providers can diagnose for gender dysphoria, prescribe hormones, and refer for gender affirming surgeries. For clinical information on prescribing hormones, you can review Sherbourne Health Centre’s Guidelines and Protocols for Comprehensive Primary Health Care for Trans Clients and the WPATH Standards of Care.
Primary care providers can access education, training and mentorship to further develop competency in gender affirming care.
Rainbow Health Ontario provides education and training to primary care providers to increase clinical and cultural competency in gender affirming care. For more information, visit www.rainbowhealthontario.ca/education-training and learn.rainbowhealthontario.ca.
CAMH’s ECHO (Extension for Community Healthcare Outcomes) project offers free CME-accredited training and consultation for trans and gender diverse care.
